For families in Bountiful

For families in Bountiful, Northern Wasatch: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.

Start with the service, date, provider and payer information on the disputed bill. Ask whether it concerns a covered claim, a separately requested noncovered service, spenddown, cost sharing or another charge. These categories have different rules; a statement that Medicaid did not pay is insufficient to establish personal responsibility.

Identify the charge and agreement before deciding responsibility

For the noncovered-service exception, ask about all four connected conditions: the provider's established policy of charging all patients for the service; notice before the service; the member's understanding of personal responsibility; and a prior written agreement identifying the service and amount. Keep spenddown, cost sharing and any valid broken-appointment conditions separate. A provider's authorization failure does not automatically become the family's private bill.

Ask the provider to explain all applicable noncovered-service billing conditions together, including the prior written service-and-cost agreement and how the member was informed. A signature does not automatically fix provider error or retroactively turn covered services into private care. Use Utah Medicaid's member assistance route for unresolved questions; do not decide to pay or ignore a bill solely from this guide. Separately confirm the next visits and how wanted daily help will continue while responsibility is examined.

Check the claim before treating the visit as private pay

Utah Medicaid's member instructions advise making sure the provider has the relevant month's card information and asking whether Medicaid was billed. Ask for the actual claim status and the explanation tied to the named service. If the issue remains unresolved, use the member information or Health Program Representative route linked by Medicaid. The financial-responsibility form identifies service, expected cost and date; it is not a general appointment of a relative to sign. A personal-care provider's participation in Medicaid does not prove every proposed task is covered.
