A Completed MyCare Assessment That the New Plan Cannot Find

For families in Euclid
For families in Euclid, Northeast Ohio: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.
If an AAA reports a completed level-of-care assessment that the receiving MyCare plan cannot find in HENS, ask the existing AAA and new plan to reconcile the actual evidence. The February 2026 guidance addresses a specified exiting-plan cohort; it is not a universal waiver of assessment.
Match the completed assessment across the handoff
Have the designated team determine whether that guidance applies to the person's history now. Identify the responsible care coordinator and confirm current services separately while the record gap is resolved. Do not create a new automatic continuity period or assume January protections still apply. Retain the actual correspondence and distinguish missing visibility from an assessment never completed.
Identify the narrow transition history before using the memo
The February 25, 2026 memo concerns members leaving Aetna or United MyCare where the AAA indicates successful completion even if HENS does not show it, including unfinished waiver enrollment. Ask the receiving plan to match that actual history with the AAA's evidence. MyCare care coordination and HCBS waiver service coordination can involve the same person or different people; the plan retains responsibility when work is delegated. If the household does not know its coordinator, use the care-management number on the member card or WRAAA's public inquiry route. A missing system entry does not itself prove an incomplete assessment.
For ordinary help at home, explore care in Euclid and contact the Northeast Ohio team. Describe the tasks the person wants help with and the arrangements already confirmed. Ask about the actual service scope and availability before relying on a care schedule.
