For families in Uniontown

For families in Uniontown, North Central West Virginia & Southwestern Pennsylvania: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.

Wanting a different hospice and choosing to end a hospice election are different decisions. Before signing paperwork, the patient or properly authorized representative should ask the agencies to explain which action the actual document records.

Ask what is being changed

The federal rules address changing a designated hospice separately from revoking an election. Use that distinction to ask the current and proposed agencies clear questions: Is this a provider transfer? Is it a decision to stop the election? What effective date and consequences apply to this person's documents?

This is not a recommendation to choose either action. The person's goals and qualified explanation should lead. A companion should not sign for the patient, backdate a form or direct an agency to terminate services.

Confirm the actual handoff

If the person chooses a transfer, ask who confirms the receiving agency's acceptance and coordinates clinical responsibility, equipment, medicines and contact arrangements. Those are agency responsibilities; a family helper can organize questions but should not arrange a technical swap independently.

Keep any county provider listing separate from an accepted admission. For example, a regional hospice listing supplies an inquiry route, not a staffed visit or a guarantee of capacity. On-call availability is also different from someone continuously present at the bedside.

Before relying on the new arrangement, confirm the responsible clinical contact and remaining daily assistance. A signed request and an actual service handoff are separate milestones, and the family needs to know what is still unresolved.

Discuss ordinary help at home

For separately arranged nonmedical assistance, our Uniontown care team can discuss the person's requested tasks and timing. Contact the local care team to ask about the actual scope, schedule and available staffing.
