For families in Waynesburg
For families in Waynesburg, North Central West Virginia & Southwestern Pennsylvania: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.
An OPTIONS arrangement paid through reimbursement or a fiscal agent should not be assumed to move unchanged into Community HealthChoices. The family needs to identify the current payment model as well as the services and worker involved.
Ask the existing care manager to name the arrangement
With the person involved, review which OPTIONS services are currently delivered, how they are paid and who supplies them. Pennsylvania's OPTIONS-to-long-term-supports transfer guidance specifically excludes Consumer Reimbursement and Fiscal Agent payment models from its transfer process and identifies the Agency Model with Medical Assistance-enrolled providers for transfer planning. The current care manager must explain how that published procedure applies to the actual arrangement; it is not proof that the same worker or payment route follows automatically.
Ask the SWPA care manager which parts can transfer, which need another arrangement and whether an agency-model bridge is appropriate. The guidance is not an instruction for the family to terminate existing help or force a particular model.
Reconcile the actual dates and providers
Ask the receiving team to explain the proposed CHC start, provider participation and authorized services. A familiar worker, submitted application or approved benefit should not be treated as a confirmed first visit under the new arrangement.
Put the actual OPTIONS end and CHC service-start information together. Identify any tasks or dates that remain uncovered and ask the responsible teams how they will be handled. Family availability and any private help should be stated honestly, not assumed to bridge an unknown gap.
Use SWPA's in-home support route to identify the existing coordination contact. Keep the person's choices central throughout. The goal is continuity based on a permitted payment arrangement and real providers, rather than expecting the old reimbursement process and worker to follow automatically.
Discuss ordinary help at home
For separately arranged nonmedical assistance, our Waynesburg care team can discuss the person's requested tasks and timing. Contact the local care team to ask about the actual scope, schedule and available staffing.
