For families in Fairmont

For families in Fairmont, North Central West Virginia & Southwestern Pennsylvania: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.

A new skilled home-health referral does not automatically replace an ADW attendant. The family needs to know which team supplies clinical services, which supplies daily assistance and how the plans fit together.

Tell both teams about the other arrangement

Inform the assigned ADW case manager about the actual referral and give the home-health team the appropriate ADW contact with the member's permission. The ADW manual provides the coordination framework; WVU Medicine's home-health information explains the clinical service route. Neither page confirms this household's admission or scheduled visits.

Ask both agencies to describe their responsibilities in the coordinated plans. A nurse's visit, for example, should not be assumed to cover all the meals, personal assistance or household routines that were previously arranged. Conversely, an attendant should not be assigned a new skilled task merely because a clinician demonstrated it to someone else.

Resolve overlaps and gaps before changing the calendar

Compare actual visit times and purposes. Ask the teams how to avoid duplicate services and whom the member contacts when instructions seem inconsistent. Keep clinical questions with the qualified team and plan questions with the responsible coordinators.

Do not cancel a familiar support visit solely because a referral was sent. Confirm the new agency's acceptance, start and scope first, then have the existing team explain any approved change. If home health ends later, revisit the daily-support plan rather than assuming all earlier visits automatically resume.

Discuss ordinary help at home

For separately arranged nonmedical assistance, our Fairmont care team can discuss the person's requested tasks and timing. Contact the local care team to ask about the actual scope, schedule and available staffing.
