For families in Fairmont

For families in Fairmont, North Central West Virginia & Southwestern Pennsylvania: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.

An inpatient stay changes where an ADW member receives care, but the family should not guess what that means for the existing case or the first visit home. Notify the assigned case manager and provider and ask them to confirm the actual status.

Establish what is on hold

Give the team the admission information it needs through its appropriate route. Ask how the stay is recorded under ADW policy, what services or scheduling arrangements are affected and whom the member should contact when a return date becomes clearer. Do not use a former calendar as proof that visits remain scheduled.

Keep the case status and the hospital's discharge planning separate. A case hold does not determine clinical readiness to leave the hospital, and a proposed discharge date does not automatically restart an attendant roster.

Prepare a real return-home start

With the member involved, ask the hospital and existing team what has changed in daily assistance, equipment or qualified-care needs. Identify who communicates the current plan to the home provider. A family member should not interpret new clinical instructions for an attendant.

Before relying on the first visit, confirm the provider, date, tasks and actual worker availability. Ask whom to contact if discharge moves or the first visit cannot be staffed. Keep willing interim help explicit rather than assuming relatives will fill every gap.

Record the confirmed return arrangement in one place. The goal is a coordinated transition with an actual start, not merely a case marked active or a familiar worker expected to appear.

Discuss ordinary help at home

For separately arranged nonmedical assistance, our Fairmont care team can discuss the person's requested tasks and timing. Contact the local care team to ask about the actual scope, schedule and available staffing.
