Fairmont DAC Programs: Coordinate the Support Plan First

For families in Fairmont
For families in Fairmont, North Central West Virginia & Southwestern Pennsylvania: use the resources and practical steps in this guide to plan wanted nonmedical help. Confirm individual requirements and current availability with the responsible provider.
A place on an activity calendar does not tell a Fairmont family who will provide a relative's required personal support during that activity. If the person already has an individualized support plan, bring that question to both the program and the team responsible for the plan before arranging attendance.
The Disability Action Center's membership page links separate client-intake and support-policy documents. Its Level of Support Policy describes specific responsibilities for people receiving IDD or Aged and Disabled waiver services. The policy connects their assistance during DAC group programs to recommendations in the person's Individualized Program Plan, or IPP.
Put the actual plan beside the actual activity
Ask the person which program they want to attend. With their agreement and appropriate authority, identify the existing support instructions and the responsible planning contact. Do not create a new level-of-care determination from the program description.
Call DAC at 304-366-3213 to discuss its current intake and participation process. Ask how staff review support needs, which information they need and how it should be shared. The provider's policy describes both on-site and off-site group activities; clarify whether the proposed session includes either or both.
Confirm who supplies the assistance
The published waiver-participant policy describes agency-provided or self-directed support. Ask the responsible care or waiver team how the actual person's plan is carried out during the proposed program. Do not assume DAC membership supplies that worker, or that a private visit automatically meets an existing authorization.
Settle travel and handoff responsibilities too. Record the activity period, who is present for the required support and who can answer a question if arrangements change. Ask what should happen if the expected support person becomes unavailable rather than improvising a different staffing arrangement.
Revisit the arrangement when something changes
The policy distinguishes people receiving waiver services from people on a waiting list and asks for notification of status changes. Let DAC and the responsible team explain which current policy applies. Joining a waiting list, receiving an approval and arranging an actual support visit are separate events.
For a separate nonmedical care inquiry, use the Fairmont care page and regional care contact. Describe the desired assistance and any existing provider instructions. Confirm service scope, staffing, payment and authorization with the responsible organizations before counting the program period as arranged care.
